Initiating Hospice Services
Eligibility Criteria:
If you have Medicare Part A, you can avail of our hospice services if you meet the following criteria:
You choose to receive care from our Medicare-Certified hospice facility.

Both your attending physician (if applicable) and a hospice physician certify that you are terminally ill, with a prognosis of living 6 months or less if the illness follows its natural course.
You complete an election statement to opt for the hospice benefit, thereby waiving all rights to Medicare payments for treatments aimed at curing your terminal illness and related conditions.
Certification and Benefit Periods
After certification, patients can elect to utilize the hospice benefit in distinct phases:

Initially, two 90-day periods are available.

These are followed by an unlimited number of 60-day periods.

From the third benefit period onwards, re-certification will require a face-to-face encounter with a hospice physician or nurse practitioner to document clinical findings that support a life expectancy of 6 months or less.
Personalized Plan of Care
All of our hospice services are guided by an individualized written Plan of Care (POC). This plan is developed by our interdisciplinary team in collaboration with your attending physician (if you have one), you or your representative, and your primary caregiver.
