Our History

Allied Hospice & Palliative Care Inc. was founded in 2016 with a vision to set new standards in the realm of hospice care. From our inception, we have been driven by an unwavering commitment to offer compassionate, high-quality care to patients and their families. This commitment was recognized early on when we achieved ACHC (Accreditation Commission for Health Care) accreditation, a significant milestone that validates our adherence to industry best practices.
Our journey over the years has been filled with noteworthy achievements, each one a testament to our dedication to excellence. We have continually evolved to meet the needs of our community, adapting to changing healthcare landscapes while staying true to our core values of compassion, integrity, and respect for the dignity of each individual we serve.

Allied Hospice is guided by a tradition of personal, clinical and technological excellence. We believe that serving patients in the community setting is the most important activity of the organization.

Philosophy

Allied Hospice is driven by the philosophy of commitment to our patients, leadership and excellence as defined below. We recognize the unique physical, emotional and spiritual needs of each person. We strive to extend the highest level of courtesy and service to patients, families/caregivers, visitors and each other.

We deliver state-of-the-art hospice services with identified centers of excellence. We engage in a wide range of continuing education, clinical education, and other programs for professionals and the public.

We strive to create an environment of teamwork and participation, where through continuous performance improvement, people pursue excellence and take pride in their work, the organization and their personal development. We believe that the quality of our human resources—organization personnel, physicians, and volunteers—is the key to our continued success. We provide physicians and environment that fosters high quality diagnosis and treatment. We maintain financial viability through a cost-effective operation to meet the organization’s long-term commitment to the community.

Our Focus

Leadership - Effective leadership is critical to the success of quality improvement efforts. Quality is incorporated with the mission statement of Allied Hospice. Leaders are present and participate in quality improvement efforts, such as participating in quality related training and committees.

System - Allied Hospice has overall system designed to meet the needs of staff and patients and geared towards achieving high quality care. Allied Hospice maintains inventory of Medical Supplies which makes for easy provision of needs promptly.

Data - Adequate data is collected and analyzed to ensure that gaps in performance are corrected to ensure optimal performance. We establish a baseline which we will compare with and measure improvement.

Outcome Metrics in Focus

We diligently track a range of patient outcomes to evaluate the effectiveness of our care. Metrics such as average length of stay, incidences of infections, patient falls, pressure sores, and patient complaints and satisfaction are among the key performance indicators we monitor to ensure quality service.

Commitment to Patient Safety
Patient safety is a top priority for us. We adhere to rigorous safety protocols, including accurate patient identification, clear and effective communication among healthcare providers, stringent management of high-alert medications, and comprehensive measures to prevent patient falls and other risks.

Regular Self-Assessment for Continuous Improvement

To maintain and improve the quality of our services, we regularly conduct self-assessments in key areas. These include clinical governance and leadership, ethical practices and patient rights, verification of professional staff credentials, and support processes for obtaining informed consent from patients and their families. We also focus on quality measures and improvements, such as clinical best practices, infection control, medication management, and overall patient safety.

Coordination of care is completed in a holistic fashion, designed to meet the physical, spiritual, emotional, and psychosocial needs of our patients. Allied Hospice captures this vision by utilizing an interdisciplinary team of professionals dedicated to our patient's unique circumstances.
